Tadalafil (Cialis — long-acting PDE5 inhibitor (~36hr half-life)) is a Small molecule C₂₂H₁₉N₃O₄, MW 389.4. Long-acting PDE5 inhibitor (half-life ~36hr vs 4–5hr sildenafil/vardenafil). Prevents cGMP degradation in smooth muscle, sustaining NO/cGMP-mediated vasodilation and relaxation. FDA approved for ED and pulmonary arterial hypertension (PAH). High PDE5 selectivity over PDE1/3/4/6.
